
The Aftercare Program is designed to provide housing assistance to very
low-income mentally, physically, or developmentally disabled persons who
would not otherwise be able to live independently.
Eligible
Applicants Must:
1) Be a head of household who is disabled or a disabled adult family member
who is dependent upon their family for care.
2) Be a current participant in a planned and ongoing program of
rehabilitation administered by one of the authorized Supportive Services
Agencies.
Aftercare Housing Assistance Applicants should contact their current
Supportive Services Agency. The Agency must complete a Certificate of
Disability and Supportive Services Form (#98042) and a Housing Assistance
Application Form (#89001) and submit them directly to the Housing Authority.
After the Housing Authority has reviewed the Application for eligibility,
the individual may be placed on the Aftercare Waiting List.
